Impact of the BioFire FilmArray gastrointestinal panel on patient care and infection control.

2020

Contains fulltext : 218876.pdf (Publisher’s version ) (Open Access) OBJECTIVES: Conventional routine PCR testing for gastrointestinal infections is generally based on pathogen related panels specifically requested by clinicians and can be erroneous and time consuming. The BioFire FilmArray gastrointestinal (GI) panel combines 22 pathogens into a single cartridge-based test on a random-access system, thereby reducing the turnaround time to less than 2 hours. We described the clinical impact of implementing the BioFire FilmArray on patients with gastroenteritis in our hospital. METHODS: Patients attending a Dutch tertiary care center (Radboud University Medical Center), from whom stool sample…

Bone marrow fibrosis and diagnosis of essential thrombocythemia

2009

However, there are serious issues to beraisedconcerningtheauthors’analysisoftheclinicaldata, thecriteriausedtodiagnoseET,andthequantificationoffibrosis.Altogethertheauthors compared a heterogenous patient database, which included311 patients evaluated for presenting features, 299 for response totherapy,361forcomplicationrates,97forprogressionoffibrosis,andfour for reversal of BM fibrosis, so no single cohort with consistentfeatures was described throughout the study of 361 patients. Forexample, for the analysis of progression of fibrosis, only 97 (12%) oftheoriginal809patientsenteredintheUK-PT1trial

Influence Diagnostics for Meta-Analysis of Individual Patient Data Using Generalized Linear Mixed Models

2014

In meta-analysis, generalized linear mixed models (GLMMs) are usually used when heterogeneity is present and individual patient data (IPD) are available, while accepting binary, discrete as well as continuous response variables. In the present paper some measures of influence diagnostics based on log-likelihood are suggested and discussed. A known measure is approximated to get a simpler form, for which the information matrix is no more necessary. The performance of the proposed measure is assessed through a diagnostic analysis on simulated data reproducing a possible meta-analytical context of IPD with influential outliers. The proposed measure is showed to work well and to have a form sim…

Missing values in deduplication of electronic patient data

2011

Data deduplication refers to the process in which records referring to the same real-world entities are detected in datasets such that duplicated records can be eliminated. The denotation ‘record linkage’ is used here for the same problem.1 A typical application is the deduplication of medical registry data.2 3 Medical registries are institutions that collect medical and personal data in a standardized and comprehensive way. The primary aims are the creation of a pool of patients eligible for clinical or epidemiological studies and the computation of certain indices such as the incidence in order to oversee the development of diseases. The latter task in particular requires a database in wh…

Patient documentation for the ultrasound laboratory

1991

The development of a PC based system for the storage of patient data in the echocardiography laboratory is described. Special design objectives were low cost and user friendliness, including an integrated special query language. After 3 years of use, the system stores 80000 diagnoses from 12000 patients without any relevant slowdown of response. Future developments concerning the communication between the database system and an echocardiographic sector scanner are discussed.

Towards an Access Control Model for Collaborative Healthcare Systems

2016

In this study, an access control model for collaborative healthcare systems is proposed. Collaboration requirements, patient data confidentiality and the need for flexible access for healthcare providers through the actual work they must fulfill as part of their duties are carefully addressed. The main goal is to provide an access control model that strikes a balance between collaboration and safeguarding sensitive patient information.

Transjugular Intrahepatic Portosystemic Shunt for Refractory Ascites: A Meta-analysis of Individual Patient Data

2007

Several randomized controlled trials have compared a transjugular intrahepatic portosystemic shunt (TIPS) with large-volume paracentesis in cirrhotic patients with refractory ascites. Although all agree that TIPS reduces the recurrence rate of ascites, survival is controversial. The aim of this study was to compare the effects of TIPS and large-volume paracentesis in cirrhotic patients with refractory ascites by means of meta-analysis of individual patient data from 4 randomized controlled trials. The study population consisted of 305 patients: 149 allocated to TIPS and 156 to paracentesis. Cumulative probabilities of transplant-free survival and of hepatic encephalopathy (HE) were estimated…

Collaborative VR for Liver Surgery Planning using Wearable Data Gloves: An Interactive Demonstration

2021

Preoperative planning for liver surgery is a critical procedure to assess a potential resection and it supports surgeons to define the affected vessels and resection volume. Traditional surgical planning systems are widely used to support the planning with the usage of desktop-based 3D and 2D visualizations. However, desktop-based systems offer limited interactions and visualizations compared to virtual reality (VR) [3]. A suitable technique to support collaboration among surgeons is required. Our previous works [1], [2] illustrate that using collaborative VR is essential to enhance communication, teamwork, and over-distance collaboration. In this work, we present a collaborative VR prototy…

Secondary technical resectability of colorectal cancer liver metastases after chemotherapy with or without selective internal radiotherapy in the ran…

2019

Background: Secondary resection of initially unresectable colorectal cancer liver metastases (CRLM) can prolong survival. The added value of selective internal radiotherapy (SIRT) to downsize lesions for resection is not known. This study evaluated the change in technical resectability of CRLM with the addition of SIRT to FOLFOX-based chemotherapy. Methods: Baseline and follow-up hepatic imaging of patients who received modified FOLFOX (mFOLFOX6: fluorouracil, leucovorin, oxaliplatin) chemotherapy with or without bevacizumab (control arm) versus mFOLFOX6 (with or without bevacizumab) plus SIRT using yttrium-90 resin microspheres (SIRT arm) in the phase III SIRFLOX trial were reviewed by thr…
